PAR-Q Gym Sessions & Membership Agreement
If you are planning to take part in physical activity or an exercise class, please accept these terms & conditions in accordance with current laws.
If you are between 15 and 69 years of age, this questionnaire will help determine whether you should consult your doctor before beginning exercise. If you are over 69 years of age and are not used to regular physical activity, you should seek medical advice before participating.
All information provided will be kept confidential.
Terms & Conditions
These Terms & Conditions (“Terms”) govern the use of the facilities, services, and membership at The Fitness Pack (“the Gym”). By becoming a member, purchasing a pass, or entering the premises, you agree to abide by these Terms.
Anyone using the gym or attending classes does so at their own risk. The Fitness Pack and sole trader Fawn Lucas-Stokes accept no responsibility for injuries sustained in, around, or before/after sessions at the premises. We are also not responsible for the loss, theft, or damage of personal belongings.
Members must:

Not plug personal electronic devices into gym equipment or sockets.

Hold their own personal insurance if required.

Train with at least one other person present where possible in case of injury or illness.

Complete a free gym induction before using the equipment.
A free induction is offered to all members. If you choose not to complete the induction and begin training independently, you accept responsibility for ensuring you understand how to use the equipment safely.
Members must report any safety concerns or faulty equipment to Fawn/The Fitness Pack immediately to help prevent injury to others.
All members are expected to:

Behave responsibly while using the gym.

Return equipment to its correct place after use.

Not move fixed/static gym equipment.

Membership Rules
Paid memberships allow entry for the paying member only. Allowing non-members access to the gym may result in immediate termination of membership.
This is a small independent business, and all services must be paid for before use to allow the gym to continue operating.
People without a valid membership are not permitted inside the gym premises, even if they are not training. Only members with approved facial-recognition access may enter the gym.
Leaving the Gym
Members must:

Switch off lights when leaving if no one else is training.

Ensure all access doors are closed both during and after use of the gym.
Payments & Access
Submitting this form does not take payment.
Payment links will be sent separately via message and processed through GoCardless.
Once payment has been received, gym access can be activated. Entry is managed through facial recognition, so members must send a selfie to 07895 226911 before access can be granted.
By completing this form, you confirm that you understand and agree to the terms above.
Membership & Payments

Membership Types — Membership options include monthly, annual, and pay‑as‑you‑go. Each membership type has its own pricing and benefits.

Payment Collection — Monthly fees are collected via Direct Debit. Failed payments may result in suspension of access until resolved.

Price Changes — The Gym reserves the right to adjust membership fees. Members will be given at least 30 days’ notice.

Cancellations — Members may cancel with 30 days’ written notice, unless otherwise stated in their membership plan.

Freezing Membership — Memberships may be frozen for medical or travel reasons at the Gym’s discretion.
Health & Safety

Medical Clearance — Members confirm they are physically fit to exercise. If unsure, they must consult a GP before training.

Assumption of Risk — Exercise involves inherent risks. Members participate at their own risk.

Injuries — Any injury or incident must be reported to staff immediately.

Equipment Use — Members must use equipment safely and follow staff instructions at all times.
Conduct & Behaviour

Respectful Behaviour — Abusive, aggressive, or inappropriate behaviour toward staff or members will result in immediate termination of membership.

Cleanliness — Members must wipe down equipment after use and return weights to racks.

Dress Code — Appropriate gym attire and footwear must be worn at all times.

Personal Belongings — Lockers are provided, but the Gym is not responsible for lost or stolen items.
Facilities & Access

Opening Hours — Hours may vary and can be changed with notice.

Access Control — Members must check in using their membership card/app. Sharing access with non‑members is strictly prohibited.

Classes & Bookings — Class spaces are limited. Late arrivals may lose their spot.

Maintenance & Closures — The Gym may temporarily close areas for cleaning, repairs, or events.
Personal Training

Approved Trainers Only — Only Gym‑approved personal trainers may offer paid training sessions on the premises.

Session Policies — Cancellations require 24 hours’ notice or the session may be charged in full.
Data & Privacy

Personal Information — The Gym collects and stores member information for membership management and safety.

CCTV — CCTV operates throughout the premises for security and safety.
Termination of Membership
The Gym may terminate a membership immediately if:

A member breaches these Terms

A member engages in unsafe or inappropriate behaviour

Payments remain outstanding after repeated reminders
No refunds will be issued for termination due to breach of Terms.
Liability

The Gym is not liable for personal injury unless caused by proven negligence.

The Gym is not responsible for damage to or loss of personal property.
Changes to Terms
The Gym may update these Terms periodically. Members will be notified of significant changes.
